Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our pledge
We are required by law to protect the privacy of your Protected Health Information ("PHI"), give you this Notice of our legal duties and privacy practices, notify you following a breach of unsecured PHI, and follow the terms of the Notice currently in effect.
How we may use and disclose your PHI
For Treatment. We use and share your PHI to provide and coordinate your care — e.g., our clinicians review your intake and lab results to prescribe and manage therapy, and we share necessary information with pharmacies (our partner pharmacies) and labs (Tasso).
For Payment. We use and disclose PHI to bill and obtain payment for services — e.g., providing information to your payment processor (our payment processor) or, if applicable, your health plan.
For Health Care Operations. We use PHI for quality improvement, clinician review, training, customer service, and business management. Example: reviewing care quality or auditing access to records.
Appointment reminders, results, and health-related communications. We may contact you about appointments, lab results availability, refills, and treatment alternatives or health benefits that may interest you.
Business Associates. We share PHI with vendors who perform services for us (e.g., our electronic health records provider, hosting, messaging) under written agreements requiring them to safeguard your PHI.
As required or permitted by law, we may use or disclose PHI for: public health activities; reporting abuse, neglect, or domestic violence; health oversight; judicial and administrative proceedings; law enforcement under specific conditions; to avert a serious threat to health or safety; for specialized government functions; workers' compensation; coroners/medical examiners; organ donation; and research, subject to legal protections.
Uses and disclosures that require your written authorization
Other uses and disclosures not described above will be made only with your written authorization, including: most uses and disclosures of psychotherapy notes; uses and disclosures for marketing; and any sale of PHI. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.
Your rights regarding your PHI
- Access / copy: inspect and obtain a copy of your records (including electronically), subject to limited exceptions.
- Amend: request correction of PHI you believe is inaccurate or incomplete.
- Accounting of disclosures: request a list of certain disclosures we made.
- Restrictions: request limits on uses/disclosures (we'll honor a request to restrict disclosure to a health plan for services you paid for in full out of pocket).
- Confidential communications: ask us to contact you a certain way or at a certain location.
- Paper copy: obtain a paper copy of this Notice even if you agreed to receive it electronically.
- Breach notification: be notified if your unsecured PHI is breached.
- Revoke authorization: as described above.

Our duties
We are required to maintain the privacy of your PHI, provide this Notice, abide by its terms, and notify you of breaches. We reserve the right to change this Notice and make the new terms effective for all PHI we maintain; we will post the updated Notice and make it available on request.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us (contact below) or with the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, D.C. 20201; 1-877-696-6775; www.hhs.gov/ocr/privacy/hipaa/complaints/. We will not retaliate against you for filing a complaint.
